Congestive Heart Failure

Our community paramedics partner with pharmacists to help reduce hospital readmissions and emergency department visits for congestive heart failure (CHF) patients discharged from the Sidney & Lois Eskenazi Hospital. Heart failure is a leading cause of hospitalization for older Americans. Approximately 25 percent of patients admitted for CHF exacerbations will readmit within 30 days of discharge. Reduction in CHF readmission rates will help improve patient outcomes and reduce costs associated with hospitalization. The Affordable Care Act established penalties for hospitals with high levels of CHF-related readmissions.

We help CHF patients manage chronic disease and prevent illness by increasing continuity of care. A community paramedic and an Eskenazi Health pharmacist provide non-emergent home visits that involve medical assessment, diet education, assistance with social needs and medication reconciliation.
